Preparation for Road Skill Test
for Adults 18 and over COURSE PROVISIONS
1. School Dayz Driving School, LLC will provide behind-the-wheel (BTW) instruction in a dual controlled
automobile that is fully insured (coverage includes the student and the instructor).
REQUIRED DEPARTMENT OF STATE INFORMATION NOTICE
This provider is required to be certified by the Secretary of State. If you have any complaint, which you cannot
settle with this provider, write: Michigan Department of State, Driver Programs Division, Lansing, MI 48918.
Completion of driver education instruction does not guarantee qualification for a driver license.
TERMS
1. The total cost of the course is $55.00 per hour with a minimum of two hours increments and is payable
during the first behind-the wheel/classroom instruction date. There is no charge for any materials or supplies
provided during the course.
2. In the event of a driving appointment cancellation not made within twenty four (24) hours prior to instruction,
a cancellation fee of $45.00 will be charged.
3. Payment methods: Cash, Visa, Mastercard, Amex, Discover, Debit, Cashier checks, & Business checks. No
personal checks
REFUND POLICY
If for any reason you decide to withdraw from the course before its completion, your refund will be based on the
following:
1. A full refund will be issued based on the following: Student will receive a full refund if withdrawal from the
course is made at least twenty-four hours prior to first instruction.
2. A partial refund will be issued based on the following: If the student does not give at least twenty-four hour
notice of withdrawal for behind-the-wheel training a fee of $45.00 dollars (forty-five) will be charged.
