SEGMENT 1 COURSE PROVISIONS

1. School Dayz Driving School, LLC will provide a minimum of 24 hours of classroom instruction, 6
hours of behind-the-wheel (BTW) instruction, and 4 hours of observation time in a dual controlled
automobile, fully insured, covering each student enrolled in the program.

2. Classroom instruction must be a minimum of 3 weeks in length.

3. BTW instruction shall not begin until the student has received a minimum of 4 hours of classroom
instruction.

4. BTW instruction must be completed no later than 3 weeks after the classroom instruction has
been completed.

REQUIRED DEPARTMENT OF STATE INFORMATION NOTICE

    This provider is required to be certified by the Secretary of State. If you have any complaint,
    which you cannot settle with this provider, write: Michigan Department of State, Driver
    Programs Division, Lansing, MI 48918. Completion of driver education instruction does not
    guarantee qualification for a driver license.

                                                                    TERMS

1. The student must be at least 14 years 8 months of age by the first scheduled day of class
(verification by birth certificate required).

2. The parent or guardian agrees to pay the amount of $270.00 which can be paid in 3 easy
installments. The
first payment of $100 is due at registration, the second payment of $100 is
due on the 2nd Monday of class, and the
final payment of $70 is due on the 3rd Monday of class.  
Failure to make a payment will result in the student not being able to attend class until payment is
made & is subject to term #9

3. Requirements to pass the course: Students must participate in class discussions, activities and
complete all work on time. Missing and/or incomplete assignments will result in automatic failure of
the Segment I course.

4. The required score to pass the STATE TEST is 70%.

5. In the event of a driving appointment cancellation, a cancellation fee of $35.00 will be charged, if
the student does not give at least twenty-four hour notice .

6. In case of a student’s absence or emergency the school’s policy is: Student, parent or guardian
must notify the School Director of the emergency and/or intended absence immediately.

7. Make-up day policy is as follows: STUDENTS ARE REQUIRED TO MAKE-UP ALL MISSED TIME.
Make-up days will be assigned upon the student’s return to class.

FAILURE OF THE SEGMENT I COURSE

9. Payment methods: Cash, Visa, Master card, Amex, Discover, Debit, Cashier checks, & Business
checks.

REFUND POLICY

If for any reason you decide to withdraw from the course before its completion, your refund will be
based on the following:

1. A full refund will be issued if a student withdraws on the first or second day of class.

2. A partial refund will be issued based on the following: If a student withdraws from class on the
third day there will be a 75% refund.

3.
THERE IS NO REFUND AFTER THE THIRD DAY OF CLASS.
