SEGMENT 2 COURSE PROVISIONS
1. School Dayz Driving School, LLC will provide a minimum of 6 hours of classroom instruction provided by a
certified instructor.
2. Classroom instruction shall not exceed 2 hours per day.
REQUIRED DEPARTMENT OF STATE INFORMATION NOTICE
This provider is required to be certified by the Secretary of State. If you have any complaint which you cannot
settle with this provider, write: Michigan Department of State, Driver Programs Division, Lansing, MI 48918.
Completion of driver education instruction does not guarantee qualification for a driver license.
For a student to participate in segment 2, verification must be received that the student has completed a
minimum of 30 hours of driving (including 2 hours at night) with a licensed parent or guardian (or parent
designee) on a level 1 license, which has been held for not less than 3 continuous months.
TERMS
1. The parent or guardian agrees to pay the amount of $50.00 (fifty dollars) which needs to be paid in full by
the first day of class.
2. Requirements to pass the course: Students must participate in class discussions, activities and complete all
work on time. Missing and/or incomplete assignments will result in automatic failure of the Segment II course.
3. The required score to pass the STATE TEST is 70%
4. The cost of materials and supplies for the class: A $20.00 (twenty dollar) refundable deposit will be required
on the first day of class to cover the cost of textbooks. However, the $20.00 (twenty dollars) deposit will be
refunded once the textbook is returned in satisfactory condition.
5. In case of a student’s absence or emergency the school’s policy is: Student, parent or guardian must notify
the School Director of the emergency and/or intended absence immediately.
6. The make-up day policy is as follows: THE STUDENT WILL BE REQUIRED TO MAKE UP ALL MISSED TIME.
However, for the Segment II six hour course, missing more than one day will result in the automatic failure of this
course. The make up day will be scheduled upon the students return to class.
7. Payment methods: Cash, Visa, Mastercard, Amex, Discover, Debit, Cashier checks, & Business checks. No
personal checks
REFUND POLICY
If for any reason you decide to withdraw from the course before its completion, your refund will be based on the
following:
1. A full refund will be issued if the student withdraws on the first day of class. AFTER THE FIRST DAY OF
CLASS THERE WILL BE NO REFUND.
